Account Deletion Policy
BeWell Grace Medical, LLC provides telehealth-only virtual care in Florida. We do not offer in-person visits or walk-ins. Patients must be physically located in Florida at the time of the visit. BeWell Grace Medical is self-pay only.
Important distinction: this website vs. the Patient Portal
This website is for general information and scheduling access. We do not collect personal medical information (PHI) through WordPress. Do not submit PHI through website forms or uploads.
All PHI and clinical communications are handled through the CharmHealth Patient Portal.
Medical record retention
If you have received care or used our telehealth services, BeWell Grace Medical, LLC is required to retain medical records for a minimum period under applicable state and federal laws and regulations. Because of these requirements, we cannot delete or remove medical records on request.
What we can and cannot delete
Medical records and portal-based clinical data
Medical records and other PHI associated with care delivered through BeWell Grace Medical are maintained in the Patient Portal and must be retained. Requests to delete medical records or clinical documentation cannot be fulfilled.
Non-medical website data
If you want to request deletion of non-medical information that may exist outside the Patient Portal (for example, basic administrative contact records), contact us using the options on our Contact page. We will review your request and respond based on what data exists and what we are required to retain.
