These Terms of Service (“Terms”) govern your use of the websites, secure patient portal, and telehealth services provided by BeWell Grace Medical, LLC (“BeWell Grace Medical,” “we,” “us,” or “our”). BeWell Grace Medical provides telehealth-only medical services to individuals who are physically located in the State of Florida at the time of the visit. We do not provide in-person care, walk-in services, or emergency care.

What Our Services Include

Our Services may include virtual medical visits with licensed Florida healthcare providers, appointment scheduling, secure messaging through our patient portal, and related administrative support. Clinical services are provided solely by licensed providers practicing within Florida and subject to applicable law.

No Emergency Care

BeWell Grace Medical does not provide emergency medical services. If you believe you are experiencing a medical emergency, call 911 immediately or go to the nearest emergency department.

Eligibility and Accounts

• You must be at least 18 years old to use the Services, or be a parent or legal guardian acting on behalf of a minor.
• You agree to provide accurate, current, and complete information.
• You are responsible for maintaining the confidentiality of your login credentials.
• You must notify us promptly of any unauthorized access to your account.

Acceptable Use

You agree not to:

• Violate any applicable law or regulation.
• Upload or transmit unlawful, harmful, threatening, or abusive content.
• Attempt to gain unauthorized access to systems or data.
• Interfere with the security or functionality of the Services.
• Use automated tools to scrape, harvest, or overload the Services.

Privacy and PHI Handling

BeWell Grace Medical does not collect protected health information (PHI) through public website forms or email. All PHI is collected and managed exclusively through our secure CharmHealth Patient Portal → in accordance with HIPAA and Florida law.

Communications Consent

By providing contact information, you consent to receive service-related communications such as appointment reminders and administrative notices. Do not use email or SMS to send urgent or sensitive medical information.

Telehealth Use and Limitations

• You understand the benefits and limitations of telehealth.
• You consent to the use of audio and video technologies for care delivery.
• You are responsible for having a compatible device, internet access, and a private location.

Payments

BeWell Grace Medical is a self-pay only medical practice. We do not accept or bill insurance. You agree to pay all applicable fees disclosed prior to receiving services.

Scheduling, Cancellations, and No-Shows

• Appointment cancellation and rescheduling policies are disclosed at booking.
• No-show or late cancellation fees may apply where permitted by law.
• We may reschedule or cancel visits due to provider availability or technical issues.

Clinical Content

Medical advice is provided only during an established provider-patient relationship. Website content and educational materials are informational only and do not replace professional medical advice.

Third-Party Services

The Services may link to or rely on third-party platforms. BeWell Grace Medical is not responsible for third-party content, availability, or privacy practices.

Intellectual Property

All content, trademarks, and software are owned by BeWell Grace Medical or its licensors. You receive a limited, non-transferable right to use the Services for personal, non-commercial purposes only.
